First Visit
Please plan on arriving to the clinic 15 minutes prior to your scheduled appointment.
Since your examination will likely involve moving around, we request that you please dress or bring in athletic clothes (shorts and a T-shirt).
The first visit will be approximately one hour that will include a history, examination, clinical decision making, and treatment if warranted.

Insurance and Billing
We accept all health insurance plans and are in network with the following PPO Plans and their subsidiaries (please contact us to confirm if your company is not listed):
Blue Cross Blue Shield, United Health Care, and Aetna.
Most insurance plans require specific deductibles and co-pays to be met prior to paying for any remaining costs. Please remember that co-pays or any deductibles may be due at the time of service.
Medical billing is often confusing for patients and their families. At the Spine and Sports Center of Chicago, we try to make it as clear and easy as possible to understand. We will work with you every step of the way.
